The Annuity Board will provide:

up to $500 / mo. disability income
contribute $35 / mo. to retirement while disabled
1/12 of value paid for each consecutive month of participation
in 12 months preceding disability
SURVIVOR PROTECTION BENEFIT Paid to beneficiary
MATCHING RETIREMENT CONTRIBUTION
LBC makes monthly contribution to your account if eligibility is met.
Every $3.00 you or your employer contribute over $52.50 / month will be matched by $1.00 from LBC
Maximum matching LBC contribution $17.50
